Acknowledgment of Risk

I, the undersigned, acknowledge that flight training and aircraft operations involve inherent risks, including but not limited to, those caused by terrain, weather, equipment failure, and human error. I voluntarily assume all risks associated with my participation in this discovery flight.

Release of Liability

I hereby release, waive, and discharge Sky Shark Flight Academy, its owners, flight instructors, and employees from any and all liability, claims, or causes of action for personal injury, death, or property damage arising out of my participation in flight activities.

Medical Fitness

I certify that I am in good physical health and do not have any medical conditions that would interfere with my safety or the safety of others during this flight.

Signature & Guardianship

By signing, I agree to the terms above. If the participant is under the age of 18, a parent or legal guardian must sign on their behalf, accepting full responsibility and liability for the minor.
